Home Care Eligibility

You or your loved one may be eligible for home care services from Functional Independence Home Care if...

• The patient needs assistance in living independently at home
• The patient is recovering from some type of acute illness
• The patient has a disability that is covered by home care services
• The patient has been discharged from the hospital and is in need of additional medical services
• The patient is recovering from some illness that requires ongoing treatment
• The patient has written orders from an attending physician who will also be providing ongoing care.
• The patient (and family) understands and agrees with needed home care services
• The patient lives in Shelby, Fayette, or Tipton Counties in Tennessee

The patient's physician may make a referral directly to Functional Independence Home Care. You or your loved one can contact us for more information at (901) 363-6046 to talk to our nursing staff for questions or concerns about home care eligibility.

• 24-Hour Critical Care Nursing
• Home Health Aides
• Physical Therapy
• Occupational Therapy

• Social Workers
